What the hell happened

A Claude Code skill for the moment you catch a failure. It walks four questions, what happened, how it happened, what the gap in the system was, and what fix is going in right now, and it does not let the last answer be a promise to be more careful.


Why I built it

I kept catching the same shape of mistake twice, which is proof the first write-up did nothing. Writing down what happened is not a fix. Ending every one of these with something a machine enforces is, and it turned my failure file from a diary into a list of gates.

Start here

  1. Download the repository as a ZIP, or clone it.
  2. Copy the folder into the skills folder inside .claude in your home directory.
  3. In Claude Code, type the skill's name right after something goes wrong.
  4. Answer the four questions, and keep the file it writes.

What it does not do

It does not find failures. Somebody has to notice one first, and the ones nobody notices are the expensive ones.
